We provide primary care/family medicine to patients of all ages. CHCNEO is a not-for-profit, Federally Qualified Health Center (FQHC) that receives funding through the Department of Health & Human Services. We currently have five clinics serving patients in three counties (Craig, Delaware, & Ottawa). Our clinics are located in Afton, Vinita, Welch, Commerce, & Jay.
